Index of a particular subfactor
No, I do not understand in any case. The definition of index to me is the dimension of $L^{2}(M)$ over $M^{G}$ module.

So, free group case can be extended it to any hyperbolic group. I understand the free group example. Can you please explain little about how to mimic the idea for any hyperbolic group? Thanks in advance.

For example all free group factors are prime @choi. One cannot write it as tensor product of two vN algebbras.

Ultrapower of an ultrapower of von Neumann algebras
For fixed ultrafilter $\omega$, $M^{\omega}$ means ultraproduct of von Neumann algebras, please look at Sorin Popa's book on $\mathrm{II}_1$ factors.
