In my experience (not at all vast) it was the authors who suggested updating their paper and I just agreed that this would be a good idea. I never had to deal with really problematic cases. But I guess if it is likely that other people will try and use an incorrect result I would write to the author once we've both agreed that a serious problem was there politely asking if they would consider updating the arxiv version of the paper and explaining the rationale. | |
