The connection between reduction theory of indefinite binary quadratic forms and the theory of continued fractions is indeed classical. Shanks infrastructure, on the other hand, gives some kind of an "almost group structure" on the principal cycle, i.e., the unit element of the class group. In addition to the references already given I'd suggest Jacobson & Williams, Solving the Pell Equation.
